Independent Care Services

Personal Care Services: These are some of the most important tasks our personal care attendants are prepared to perform. They can assist with bathing, toileting, dressing and other tasks of daily living, with the utmost respect and sensitivity.

Homemaking Services: A clean home is a healthy home! Our homemakers can provide light housekeeping, laundry and ironing, changing linens, grocery shopping, meal preparation and errands.

Transportation: If it’s a hospital pickup, visiting a friend, a shopping trip, or an appointment, our caregivers can provide the comfortable transportation needed for those unable to drive.

Respite Care: Respite allows the caregiver an opportunity to take a break and provide some downtime to lessen the burden of caring for a loved one. We provide individualized respite care based on the needs of the individual and their caregiver.

Companionship: Whether it is conversation, playing cards, reading, taking a walk, or just having someone to visit with, companionship can help make for a happier, healthier life. Having a companion can decrease the feelings of isolation and loneliness – let our friendly staff enrich your life.

Care Management Services

Care Coordination: Our Registered Nurses and Social Workers coordinate the care of individuals who face substantial independent living challenges due to age, illness or disability. We can develop a plan for the patient’s needs, then mobilize our resources to enhance health, safety and quality of life, while coordinating those resources through ongoing monitoring.

Transition Services: We can create a comprehensive service plan for those transitioning to an alternative living situation. We coordinate the transition of individuals from a facility (i.e. nursing home or hospital) to home, or can assist family members to move a loved one needing a higher level of care, into into a senior apartment, an assisted living facility or a nursing home.

Wellness Checks: Put your mind at ease: our caring staff can make phone calls to an elderly parent for medication reminders, wake-ups and general wellness checks.

In Home Safety Assessments: We want you and your loved ones to live safely at home. We perform an in-depth assessment of safety issues, recommend safety equipment and make referrals to community services.

Monitoring Diet/Eating: Many seniors living at home are undernourished. Our caregivers can help by monitoring food & fluid intake, preparing nutritious meals or assisting those who need help eating.

Activity Planning: Everyone has a hobby they love, an activity they enjoy, or an important routine they must follow. Our staff can help by planning and assisting with these activities so you or your loved one can maintain independence, keep minds active, and have a little fun!

Money Management: At some point in time, an older adult may need assistance with paying their bills. Our care managers can assist with check preparation, bill paying, and medical claim filing.

Equipment and Supplies

In conjunction with our in-home safety programs, we provide recommended equipment and supplies for Fall Prevention, Baby Safety or Medication Management. Equipment is available for purchase or rent including:

Computerized Medication Dispensers: Deliver medications in the correct doses at the correct time and call caregivers for missed medications.

Personal Emergency Response System (Lifeline): An easy to use medical alert system designed to reduce the risk of living alone. In the event of a fall or emergency, help is available at the push of a button.
