Welcome to your best source for HIV and AIDS Services. Our HIV/AIDS Division has several programs serving individuals with HIV/AIDS and their families. We have been providing our HIV/AIDS services throughout the north state for more than 25 years. Program services may vary from county to county.

Programs
AIDS Medi-Cal Waiver Program: To be eligible for this Case Management program, you must have full scope Medi-Cal, have a physician confirmation of HIV, meet the Nursing Facility Level of Care, and have a primary physician willing to be responsible for your medical care.
Care Model Program: This program provides support for critical HIV/AIDS services. Services under this program include:
- Medical Case Management
- Emergency Financial Assistance
- Medical Transportation
- Oral Health Care
- Out-patient Ambulatory Medical Care
AIDS Drug Assistance Program: This program allows for eligible individuals to obtain their HIV/AIDS medications at no cost. Covered drugs must have been proven to improve the quality of life or prevent and treat HIV/AIDS diseases.
Our Registered Nurse and Social Work Case Managers have in-depth experience to assess and evaluate which specific programs will meet the individual needs of those with HIV/AIDS.
Medical Case Management Services
Case Management by a Registered Nurse: Comprehensive nursing case management services are provided on an ongoing basis to assist with the complexities of the HIV/AIDS disease process.
- Face to face nursing assessments
- Monitor medical needs
- Provide nutritional counseling
- Medication teaching
- Provide information about diagnosis
- Physician Referrals
- Answer questions
- RN advice nurse available 24 hours per day
Case Management by a Social Worker
Comprehensive social work case management services are provided on an ongoing basis to assist with the social, financial and emotional aspects of an individual with HIV/AIDS.
- Face to face psychosocial and financial assessments
- Benefits counseling
- Advocacy with Social Security, Disability and Social Services
- Budget Planning
- Assistance with legal issues/housing
- Referrals to counseling, community resources and Support Groups
- Assistance with medical transportation and food
Supplemental Services
Support Group Meetings: A Social Worker/Therapist presides over support groups for those with HIV/AIDS. Our support groups are open to clients, non-clients and family members and are held regularly in both our Chico and Redding offices. Please call for days and times.
Individual Counseling Sessions: Some of our clients prefer privacy and benefit more from one-on-one counseling sessions rather than in a group setting. Counseling is available on an individual basis dependant upon client needs and program enrollment.
Health Insurance Premium Payment Program (HIPP): This program can assist with premium payments for various insurance policies that provide HIV-related health care. Our social work Case Managers can assist individuals to determine if they are eligible for this program.
Housing Referrals: Our staff can make referrals for housing through the “Housing Opportunities for Persons with AIDS” Program.
Speakers Bureau: We have experienced staff available to speak at local events and businesses.
